Direct Primary Care is a model of health care that is focused on the provider-patient relationship. Your provider contracts with you, not your insurance company, to provide comprehensive and personalized primary care that is convenient for you.  By providing primary care services for a monthly fee paid directly to your provider, there are no unexpected or hidden costs associated with your care: no deductibles, no co-insurances, no co-payments.  Patients get direct and unrestricted access to their provider by phone, email, text, messaging or video chat. 

We do not accept or bill insurance. Membership services are offered for a low monthly fee. This allows us to provide personalized healthcare without the hassles of insurance companies. Care is based on your needs, not what the insurance company demands. No unnecessary appointments. No restrictions. By eliminating the demands of insurance companies, we are able to keep our patient panel number low, allowing us more time to spend with our members.

Direct primary care does not replace health insurance. At Meadow Mountain Health we provide quality care without insurance. While most of your healthcare needs may be covered through the DPC membership, it is a good idea to have insurance coverage for other needs such as major medical expenses, prescriptions, testing, and procedures.  

In order to keep our prices affordable, Meadow Mountain Health does not have a full time office at this time.  We do share office space with Holistic Rhythms Wellness & Sleep Center in Oakland Md.  For memebership plans, the initial visit will be conducted at the office. Subsequent visits can be conducted via telehealth or in your home (or other appropriate locations).  When necessary or preferred, subsequent visits can be held in the office.

Your primary care membership fee allows you to have unrestricted access to your provider. A personalized health care plan will be created with a focus on wellness and prevention. You will have unlimited number of  acute and chronic care visits. We offer same or next day appointments in the form of telehealth, office visits and home visits. Visits at your convenience including early morning, evening and weekend appointments. Routine physical exams, pre-employment exams, sports physicals, school physicals, and DOT physicals are included in the membership. 

Your acute care membership fee covers all family members who reside in your home. Each family member will have unrestricted access to your provider for acute care needs only. This membership does not include primary care services.  

Each family member will be seen for an initial history and physical. Patients have unlimited access for uncomplicated acute care visits. We offer same or next day appointments in the form of telehealth, home visits, and office visits. Visits are performed at your convenience including early morning, evening and weekend appointments.  If the problem is more complex, the patient may be referred to another facility for further evaluation and treatment. 

Primary Care, routine physical exams, pre-employment exams, sports physicals, school physicals, and DOT physicals ARE NOT included in the membership. 

Your acute care plan covers all family members who reside in your home. The acute care plan does not provide unrestricted access to your provider. This plan does not include primary care services.  

Each family member will be seen for an initial history and physical, with a one time fee of $100 per household. Family members will then be able to schedule uncomplicated acute care visits for a set fee.  We offer same or next day appointments in the form of telehealth, home visits, and office visits. Visits are performed at your convenience including early morning, evening and weekend appointments.  If the problem is more complex, the patient may be referred to another facility for further evaluation and treatment. 

Primary Care, routine physical exams, pre-employment exams, sports physicals, school physicals, and DOT physicals ARE NOT included in this plan. 

Your single acute care visit is for those patients who do not choose to participate in a membership plan.  You do not have unrestricted access to your provider or unlimited visits. This is strictly individual, one time uncomplicated acute care visits. 

We offer same or next day appointments in the form of telehealth, home visits, and office visits. Visits options include early morning, evening and weekend appointments when available.  If the problem is more complex, the patient may be referred to another facility for further evaluation and treatment.
