This plan covers individual patients or the entire family.

Adults  $65.00 per month

Child    $25.00 per month**

** We will not charge for more than 2 children per family unit. For example, a couple with 4 children will pay $180 per month. A single parent with 3 children will pay $115 per month.

Membership fees are paid monthly. 

This plan covers each family member within the home. 

 $50.00 per month

6 month contract required

Each family member will be seen for an initial history and physical. Patients have unlimited access for uncomplicated acute care visits, including telehealth and home visits.

$100.00 Initial visit to meet the patients

$50.00 per telehealth visit

$100.00 per in person visit

These patients WILL NOT have access to membership benefits such as unlimited access to the provider. 

This is for individual patients who do not participate in a membership plan.

  $125.00 per visit

 These patients WILL NOT have access to membership benefits such as unlimited access to the provider.
